licensure in North Dakota.

for ND licensure for applicants who previously
held a ND license.
The requirements for physical therapists interested in applying for licensure with North Dakota & taking the National Physical Therapy Examination (NPTE) are as follows:
Complete the online license application as a Physical Therapist or submit a completed application to the ND Board of PT Office.
Payment can be made online using a credit card or enclose a check for $200 payable to the ND Board of PT, for the
non-refundable application fee.
Download, complete and return the Juris Prudence Examination. (This is an open book exam which can be completed by using up the Rules & Regulations and ND PT Practice Act in the right menu bar.)
An official transcript, indicating completion of an accredited physical therapist program, must be sent directly from the school of
graduation. If an official transcript is not available at the time of application, a letter from the school director indicating
graduation will be accepted for approval to sit for the NPTE.
Applications for the NPTE must be done online through the FSBPT, and can be completed at anytime. You will not be cleared to sit for
the NPTE until the North Dakota Board of Physical Therapy notifies the FSBPT that they have received all of your application
materials and payment. Please refer to the FSBPT website at www.fsbpt.net/pt for information on that process.

Eligibility for a permanent license is only possible when a passing score on the NPTE and an official transcript indicating
graduation have been received.

in which a license is held/previously held. Please request that this completed form be sent directly to the ND Board of PT.
Contact the FSBPT to have your NPTE scores forwarded directly to ND Board of PT.
Enclose letters from the department supervisor or human resources from the last two places of employment, verifying employment.
