License Renewal

Renewal Fees
  1. Physical Therapists = $100.00
  2. Physical Therapist Assistants = $60.00
Make sure to have the following information before you begin the online renewal process:
  1. Your last name as it appears on record with the Board (If you need to verify, please click here to look up that information.);
  2. Your social security number;
  3. Your PT/PTA license number (If your license number is 3 digits, please add a preceding zero. For example, if your license number is 211, type 0211.);
  4. A current Visa or MasterCard account;
  5. The expiration date of the Visa or MasterCard account.
All information entered will be encrypted, so your data will be transmitted safely and securely.

Once you have the above information in front of you, read the instructions below and begin the renewal process. To begin the renewal process fill in the fields with the information requested and click the "Enter" button. You will have a chance to review all information for accuracy before final submission. While you are in the renewal process please do NOT use your browser's "Back" button. If at any time you wish to exit the renewal process, please click the "Exit" button at the bottom of the page.

Renewal Requirements
  1. Physical therapists and physical therapist assistants must complete at least 25 contact hours of continuing education every 2 years.
  2. New graduates are exempt from reporting continuing education hours for the period following graduation.
Name Change
If you have had a name change, you may change it online.

Address/Email Change
If you have had an address or email change, please follow the online prompts and update your contact information.

Late Renewals
Online renewal must be completed by January 31st of the current year. After January 31st an additional $50.00 late fee will be assessed.

Please call the board if you have any questions about your application.

Choosing exit will take you out of the renewal process.
