Mailing List Request

The North Dakota Board of Physical Thereapy now provides a mailing list online. To obtain a copy of the mailing list you will need to pay a $25 fee. The mailing list will be available on the receipt page after the payment has been submitted. The file will be in an Excel CSV format.

DO NOT Close or Refresh the browser or you may be charged twice or the payment may not be submitted.
